Please Read the instructions below thoroughly before submitting your duplicate request form.

Trophies $85       (Trophies may be requested for Gold, Silver, or Bronze Awards)
Certificates $25  (Certificates may be requested for Gold, Silver, or Bronze Awards)

• Fill in the request form below, once for EACH specific duplicate request
• If you are requesting more than one of the EXACT same thing, you only need to fill out one request
• Any change of information, such as an individuals name credit, will require a new form
• If you anticipate requesting more than one award duplicate, please note this in the appropriate field

Once you have entered a request you will be directed to a confirmation of submission page; please print this page for your records. When we have received your request(s), you will be emailed an invoice with a total amount and a confirmation of all award duplicates requested. This email will include an invoice number and a link/URL to the payment page.

Please be sure to proof read your entries before submitting. If there are diffrences between our records and the duplicate request form we will assume that the request form includes a correction. We are not responsible for typographical errors in data entry. The conformation page is your final proof copy. If you find a data entry error, email us with the correct information immediately.
